Standard +hp runes will take you a long way. It all depends on your build though.

There's an entire subforum for paragons: http://www.guildwarsguru.com/forum/paragon-f239.html
The sticky in that forum says:

~General purpose spear Build:
spear: Furious Spearhead (double adrenaline 10%) ,15^50 damage inscription (15% extra damage while above 50% health) ,and spear grip of fortitude (+30hp)
shield: (either motivation or command) Luck of the draw inscription (-5/20%) with +30hp handle.
Armour: centurions insignia with minor runes on all attributes exept the highest invested attribute (spear...which would get the superiour rune on a spear headpeice) the remaining slot should be a rune of vigor (the highest you can afford)
You can also use pvx, all builds have an equippment section.
This is for a pretty popular imbagon build:

Centurion or Survivor Insignias.
Furious Spear of Fortitude/Defense
Command Shield of Fortitude
Tactics Shield of Fortitude if using "To the Limit!".
High energy set if DP drops your energy below 25.

Note; As you will pretty much always be running at least 3 different attribute lines at similiar power as a Paragon (around 10-12 each), it tends to be wiser to invest in 2x Major Runes than 1x Superior Rune IMO, that way you end up with an evener spread and come off +5HP better off.

Personally, I only use one Major Rune or none at all in most cases. I do have a swapout for 2x Majors when I want to meet skill breakpoints though.

The other thing to remember with Paragon, is that, unlike other professions, you are very much a support class (probably have some of the worst DPS in the game as far as PvE is concerned due to lack of AoE attacks), meaning that whereas with most other classes, your offensive attribute is often your highest stat, with Paragons this should generally be less the case (PvP is a different case however), and you'll probably be running Spear at 9-10 more often than you'd think, and very rarely above 12 (twelve will generally just be to meet breakpoints on skills like Burning Spear).

Kendil pretty much has it said, but it'll all eventually fall down to what builds you run.

In time, you're going to want an armor set for every rune/insignia setup possible. For now, get headpieces of every attribute with a + rune of your choice to that headpiece's respective attribute so you can test the waters with different builds. I'd suggest staying away from Major/Superior runes early on...

KotCR was pretty much right on the money with saying you'll have to always specialize in 3 attribute lines (which is true for most classes, but it's more complex as to why it sucks particularly for the paragon), which is applicable to 90% of current paragon builds (Martial Weapon, Leadership, Shield Attribute line). Though, you are free to extend your views on this as far you wish (2 attribute investment, Searing Flames Paragon lulz).

If you're running Paragon in PvP (LOL), for general use, you want to get Spear Mastery as high as possible, 14-15; 16, if you're feeling moody. You want to at least match the minimum requirement for your shield, and as for Leadership... well, this value fluctuates greatly for each build/situation. Really, just count the number of party members you can have max in that arena, or whatever amount you'll usually have around at any given time while in said arena... so your Leadership trait could be anywhere from 2-12 (never go higher than 12; you shouldn't need it any higher)...
